If you are travelling abroad, you will need to read our "Advice for travellers" letter below then complete the Travel Immunisation form before you contact the practice to arrange an appointment.  You MUST return the  completed form to us 1 week prior to your appointment and any vaccinations requiring payment must be paid for in advance.

It is your responsibility to ascertain which vaccinations (and, if relevant, anti-malaria tablets) you need, and also to inform yourself fully of recommended travel safe precautions.  To do this, you should visit the NHS Fit For Travel website at and read the information relevant to the country you are visiting.
